GreatFirstWeek — collecting stories about transitions into care, education, and support settings
When someone moves into care, school, employment or support, the team is often piecing together who they are and what they need on the day they arrive — even though that information already existed somewhere. It was in a report, a plan, a phone call, or someone's head. It just never reached the people who needed it in time.
We are collecting stories of what that first week actually looks like — for the person arriving, and for the team responsible for them — across every transition setting. The survey takes about three minutes, and it helps us build something that fixes it.

This work is led by UNPLEXi Pty Ltd (Dr Anna Moran). We collect responses to understand how transitions into care, education and support can be improved, so we can build something that helps. If you choose the lived-experience path, your response is anonymous — we do not ask for your name or contact details. We only use stories in anonymised form, with no identifying details, unless you give separate permission. You can ask us about how we handle information at any time by emailing anna@unplex.com.au.
